Campus Meeting Room Request

Please use this form when requesting space on campus that does not relate to a class or make up exam.

If you need to schedule a class or make up exam, please contact Carrie Schwab.

When listing the meeting title - no abbreviations - please spell out the full name of the group. This will help when guests arrive and they need assistance.

A count will be needed if outside guests are invited for a meal in the Refectory, so the proper amount of food may be prepared.

Room Set-up
(needed if using Benbow Hall or changing a classroom configuration)

Explain when Ohter Setup was selected above or provide additional setup instructions.
Media Services & Technology
Indicate what additional equipment or technology you will need for your event
Hold down the control (ctrl) key to select multiple items
Food Services
Indicate if food service is required for your event. 
Our Hospitility Coordinator will assist you in selecting food service items
Guest Housing
If you will require Guest Housing, Please complete the following:
Check-in time is 4:00 PM or later
Check-out time is 11:00 AM
(Who can provide additional information about this event?)
Email address of the person who can provide additional information and will be notified when the meeting is confirmed
Attach a document to further explain your event needs or to provide a desired room layout.  File formats accepted are doc, txt, rtf, pdf, ppt, xls.
